​Prof. James D. Luketich:科技的进步为食管癌治疗带来曙光

编者按:在2017国际胸外科学术大会(CITSAC)暨第五届国家癌症中心学术年会(NCCAC)上,来自美国University of Pittsburgh Medical Center的James D.Luketich教授就“微创食管切除术”作出了精彩的演讲。借此机会,我们有幸采访到了Prof. Luketich,为我们分享了食管癌治疗的最新进展以及对未来的展望,此外,对于青年外科医师,Prof. Luketich教授结合自身经历,也给予了十分宝贵的建议与意见。

Dr.Luketich’s areas of interest include thoracic and esophageal surgery. Theseinclude gastroesophageal reflux disease, management of hiatal hernias,management of giant paraesophageal hernias, and other disorders of theesophagus, including diverticulitis, strictures, and many others. He hasparticular expertise in the multi-disciplinary management (combinedchemotherapy/radiation and surgery) of esophageal and gastroesophageal cancer,Barrett’s esophagus, and lung cancer. He performs numerous complex operationson disorders such as these using minimally invasive techniques, for which hehas become world renowned, performing over 1,500 minimally invasiveesophagectomies.

Dr.Luketich is the principal investigator or co-investigator on numerous researchgrants and has published over 250 peer-reviewed manuscripts, 150 invitedreviews, textbook chapters, and editorials, and 350 abstracts. He is frequentlyinvited as an honored speaker around the world to present topics that relate tohis research and minimally invasive techniques.

Dr.Luketich serves on the editorial boards of the Annals of Surgery, Journal ofThoracic and Cardiovascular Surgery, and the Journal of GastrointestinalSurgery. Dr. Luketich is a Fellow of the American College of Surgeons, and is amember of many prestigious surgical societies including the American SurgicalAssociation, the American Association for Thoracic Surgery, and the Society ofThoracic Surgeons, among many others.

Priorto joining UPMC in 1995, Dr. Luketich was a senior instructor in surgery at MemorialSloan-Kettering Cancer Center in New York. He has additionally held academicpositions at Cornell University Medical College and the University ofPennsylvania.

Dr.Luketich received his medical degree from the Medical College of Pennsylvaniain 1986 and completed his general surgery residency in 1992 at the Hospital atthe University of Pennsylvania, where he was the chief resident of surgery from1992-1993. He completed his cardiothoracic and thoracic training at New YorkHospital, Cornell Medical Center, and Memorial Sloan-Kettering Cancer Center.

对于近十年食管癌治疗和管理方面所取得的进展,Prof. Luketich认为最大的进步在于通过微创手段使得食管癌发病率和死亡率都明显下降。近年来食管癌的发病率从10%左右下降到了1%,这是依赖于微创外科技术所取得的重要成果。

近十几年来,随着科学技术的发展,食管癌手术方式也发生了翻天覆地的变化。Prof. Luketich表示,多种新型设备如超声刀或声波剪刀,使我们可以迅速地分离血管而毫无并发症,从而实现无血外科手术。机器人在癌症治疗中心的广泛应用对于食管癌手术的发展同样影响巨大,Prof. Luketich相信在不远的将来,机器人可以进一步降低发病率。

对于未来食管癌治疗的发展,Prof. Luketich认为,科学技术的进步将会继续在手术治疗层面发挥巨大作用,我们也将会看到用于治疗更多疾病的新型机器人与其它技术的出现与广泛运用。对于今后食管癌的治疗,Prof. Luketich认为医药公司将会扮演重要角色(研制出更好更有针对性的治疗),而免疫疗法也将带来新希望(帮助“打开”病人自身的免疫系统以抵抗疾病)。对于未来,Prof. Luketich的态度是积极的,并相信免疫疗法、化疗、手术治疗等方面将会有更多的新进展。

对于青年外科医师, Prof. Luketich以专业导师的身份,鼓励他们不仅应获得最好的基础培训,更应注重寻求在出色的医院机构获得先进的专业培训。
